Before you begin

This questionnaire measures how psychologically safe you feel across three areas: your social connections, your capacity for compassion, and your bodily experience. It is grounded in polyvagal theory and was developed by researchers at the University of Strathclyde and the Traumatic Stress Research Consortium.

There are no right or wrong answers. Higher scores reflect stronger feelings of safety. Simply respond based on how you have been feeling recently.

The questionnaire takes about 5 minutes to complete. Your responses will be shared only with your practitioner.

What do these scores mean? Higher scores reflect stronger feelings of psychological safety. The Social Engagement score captures how safe and accepted you feel in your relationships. The Compassion score reflects your capacity to connect with and care for others. The Body Sensations score captures how calm and settled your body feels. Together, these give us a picture of how your nervous system is evaluating your environment. There are no good or bad scores. We use them as a starting point and to track how things shift over time as we work together.
